The Boston Passport Agency will be on hand to offer a seamless application process, including routine and expedited processing, for anyone seeking to apply for a new, renewed, or minor passport.

Here is what you must bring with you:  

  • Completed DS-11 application: Fill out the U.S. State Department's Form and bring a printed copy with you. DS-11 forms must be unsigned, and the applicant must appear in person.
    • If you are renewing a passport, complete the DS-82 application form here.
    • Applying for a Passport as a 16-17 Year Old? For any adult age 16 or 17 applying for the first time or renewing a passport issued under age 16.
      • You must apply in person using the DS-11 application form for your first adult passport.
      • You must show one parent or legal guardian is aware you are applying.
      • Passports for applicants 16 and older are valid for 10 years.
    • Applying for a Passport for a Child Under 16? Both parents or guardians must give their approval for a child to receive a passport and must be present with the child when applying.
      • If one or both parents or guardians cannot be present with the child, you will need to show us more documents.
      • You cannot renew a child's passport using DS-82 application form. Please use the DS-11 application form.
      • Passports for children under age 16 are valid for 5 years.
  • Citizenship evidence: Original certified birth certificate, consular report of birth abroad, naturalization certificate or previous passport and photocopy of citizenship evidence.  For renewals, bring your previous passport which is expired less than 5 years.
  • Identification: Government-issued photo identification and a photocopy of the front and back (for example driver license, state ID).
  • Passport Photo: One 2-by-2 inch passport photo. If you do not have a passport photo, staff from the West Warwick Public Library will be on-site to take one. Passport photos will cost $13.00 payable directly to the West Warwick Public Library through cash, check, money order or credit card.
  • Payment: Check or money order ONLY payable to the U.S. Department of State. See travel.state.gov for fees.
